ESSENTIAL DUTIES AND RESPONSIBILITIES
Supervises the unit under the direct supervision of the Director of Nursing. Act in the capacity of leadership on the unit, to direct staff on assessment of residents, to evaluate care and personnel issues, and develop solutions to identified issues and problems.
MAJOR DUTIES AND RESPONSIBILITIES:
The Fountainview Mission Statement and facility philosophies will be known, owned and energized by every staff member.
Participate in all aspects of the Facility CQI program.
Perform or oversee daily rounds to assess residents to determine if their medical and psychosocial needs are being met.
Monitor medication administration passes to ensure accuracy and timeliness of administration and to help identify areas of concern or staff in service needs.
Oversee the scheduling of CNA assignments, breaks and lunch breaks to ensure the needs of the residents are being met; assess work assignments and schedule to assist staff in completing their assignments.
Coordinate and interact with the interdisciplinary team and families to ensure the needs of the residents are met.
Evaluate infections on the unit to identify areas of concern, patterns of infections and help develop strategies to reduce infections.
Assist the charge nurses with entering physician orders.
Assist with admissions, discharges and any risk assessments that identify resident needs for care plan development or updating of care plan.
Conduct clinical conferences with staff based on needs identified through compliance rounds.
Maintain and review medical charts and documentation in accordance with state and federal regulations and to help identify documentation needs; educate staff on documentation standards.
Monitor compliance of staff with state and federal guidelines.
Evaluate and assess residents for risk factors, delegate assessments to staff; help educate staff on risk factors and documentation of risk factors.
Perform staff evaluations of performance.
Coach, mentor, teach, and counsel staff relating to performance as well as discipline staff relating to job performance.
Serve and participate in facility committees as assigned by the DON and or Executive Director.
Delegate to charge nurses and other personnel the authority, responsibility and accountability necessary for their assigned duties.
Attend and participate in continuing educational programs and seminars designed to keep you abreast in changes in your profession and relating to Alzheimer’s Disease and research.
Train staff on utilizing the documentation systems such as AHT Smart Charting, E Chart, and all aspects of the electronic medical record, daily validation that charge nurses are documenting in the system and poling the system to ensure the CNAs are completing their daily documentation.
Teach and train staff in resident care as needed.
To assist in all facets of resident care as needed.
Provide supervisory and or nursing coverage of pavilions as needed.
Other duties as assigned by the Director of Nursing or Executive Director.
Must be proficient in writing and speaking English and have the ability to communicate with the residents, families and staff.
