Examples of Duties Performed:
- Plan, organize and direct wound, ostomy and continence services that meet the patient and center’s needs and coordinate such services with the Director of Nursing Services.
- Develop and direct the implementation of wound, ostomy and continence program procedural guidelines.
- Monitor the center wound care program via the electronic health record.
- Attend and participate in patient assessment and care planning meetings as necessary including Tele-medical consultations.
- Provide center on-site visits.
- Instruct other health team professionals and family members in the methods of assisting the patient to improve or correct compromised skin integrity on center visit.
- Work with the organization team to conduct QAPI reviews and other activities as needed.
- Orient new treatment nurses in the centers to their role.

Requirements |
- A Registered Nurse license in good standing in the state of Georgia and at least two years of experience in wound care in a hospital, skilled nursing care center or other related medical center setting.
- Bachelor of Science degree and current national certification in wound, ostomy and continence preferred.
- Basic knowledge of principles and practices of long-term and post-acute management and administration.
- Ability to communicate effectively both orally and in writing.
- Must be able to establish and maintain effective inter-departmental communication to provide positive working relationships.
- Must be able to travel throughout Northeast GA and state as required.
